Make a Difference

Care Planning Partners has a full-time Ambulatory LPN opening at 8101 Clearvista Drive Suite 200 in Indianapolis, IN. The Ambulatory LPN works hand-in-hand with the nurse practitioner to achieve quality outcomes for the patient. The Ambulatory LPN performs direct and indirect patient care under the supervision of the NP and collaborates on the plan of care. Direct patient care includes rooming patients and completing vital signs. Indirect patient care includes completing cognitive screenings, obtaining health histories, order transcription, documentation, and reinforcement of patient and family education. The Ambulatory LPN also collaborates with other team members to perform other unit services as directed.

Exceptional Skills and Qualifications

Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience. (Required)
Previous experience in medical office or health care setting. (Preferred)
Licensed as a Licensed Practical Nurse (LPN) by the Indiana Professional Licensing Agency (IPLA)
